Why are some therapists not on my insurance approved list?

Insurers set a maximum fee they will pay a therapist. To appear on the approved list, a therapist must agree to work at or below that cap. Many experienced, senior clinicians cannot run a sustainable practice at those rates, so they are not on the list. The list is shaped by economics, not by clinical suitability.
